Faculty Disclosure of Relevant Financial Relationships
SDSI requires all faculty members to disclose all relevant financial relationships with any commercial interest(s). Disclosure includes documentation of any relevant financial interests within the past 12 months, the name of the commercial interest, and the nature of the relationship. This information is disclosed in a handout included with course materials. The handout also lists those individuals with no financial relationships to disclose. All conflicts of interest potentially impacting educational content have been resolved prior to this course.
All meeting faculty must complete a faculty disclosure form prior to course participation. We prefer faculty members to complete this form electronically. In Februray, faculty will recieve an email with instructions for completing the form electronically. We encourage faculty to submit the disclosure form as soon as you recieve the email with instructions from us. Uploading Faculty Handouts and Course Materials
SDSI has chosen Omnipress (www.omnipress.com) to assist with handout collection and printing of the course syllabus. Uploading instructions have been mailed to you. All handouts must be uploaded by April 23, 2010. Handouts are an essential adjunct component of the learning process. SDSI believes that, as educators, it is our professional responsibility to provide this resource to our learners. Therefore, handouts are mandatory for all lecture sessions.
SDSI expects all CME events to be presented in accordance with ACCME, U. S. Food and Drug Administration (FDA), AdvaMed Code of Ethics, and regulatory and legal standards.
Content Guidelines
- All recommendations involving clinical medicine must be based on evidence that is accepted within the profession of medicine as adequate justification for their indications and contraindications in the care of patients.
- All scientific research referred to, reported, or used in CME in support or justification of a patient care recommendation must conform to the generally accepted standards of experimental design, data collection, and analysis.
- SDSI discourages discussion of off-label use of any product. Any faculty member desiring to discuss the off-label use of any product must disclose that the FDA has not approved use of the product in the manner discussed and/ or that the product is still investigational.
- SDSI further discourages any discussion regarding billing and/ or reimbursement issues. A faculty member who discusses reimbursement or billing issues must receive prior approval from the course chair. If approved, the faculty member must disclose that the company is not responsible for information being provided.
- All information presented in CME activities must be based on factual data and not incorporate personal or commercial bias.
- Content and format of CME must support improvements in healthcare and not a specific proprietary business interest.
- CME activities must give a balanced view of therapeutic options.
- Handouts cannot contain any advertising, trade names/ registered trademarks, or product-group messages. If use of trade names/trademarks is necessary, trade names from several companies must be included.
- All educational material must comply with legal and regulatory standards.
- Protected health information and other patient identifiable information may not be disclosed during educational presentations.
- Handouts should include a bibliography and a post-operative protocol (as appropriate to lecture content).
Submission Format
- Please submit your handout electronically in one of the following formats: Microsoft Word Document (*.doc), PowerPoint (*.ppt), or Adobe PDF document (*.pdf).
- If submitting MS-Word or Adobe PDF, please format page size to 8.5” by 11” with all margins of one inch or more.
- Please include all photos/ illustrations/ figures within your document. 600dpi resolution will reprint better than 300.
